A Study of Nurse Manpower Utilization in University Hospital

Abstract
|
|
|
As for the expansion of nursing service in University Hospital, the forcast of man-power and it´s appropriate utilization is the most important concern to the nursing administrators for maintaining quality patient care and reasonable hospital cost.
The purpose of this study is to provide available data to be utilized nursing staff effectively according to patient classification, nursing activity analysis and patient care needed.
This study was conducted on the hospitalized patients and nursing personnels in Seoul National University Hospital from October 1 to December 30, 1979.
The results are as follows;
1. Distribution of Patient Classificaton
(1) The average number of inpatients was 693 and was not different from Monday to sunday remarkably.
(2) The distribution of acutely ill, moderatly ill, and mildly ill patients was 17. 2%, 33.2% and 49.6% in medical, surgical and pediatric nursing unit. Surgical and pediatric nursing unit was showed more class ji , acutely ill patients.
2. Analysis of. Nursing. Personnel´s Activities.
(1) Nursing personnels spent the time. of 42. 7% for direct nursing activities, of 47.9% for indirect nursing activities in medical nursing unit.
Nursing personnels spent the time of-;38.8% for direct nursing activities, of 56.5% for indirect nursing activities in surgical nursing unit.
These were patient education, eating a meal and personal time, was showed less than idial duration.
36 (2) The head nurses worked average 8. 3 hours in duration. She-spent the time 70. 1%, 69.3 for indirect nursing activities in medical and surgical nursing unit respectively. There were coordination to other department and paper work for the patients.
(3) The staff nurses spent the time 58% for direct nursing activities, 34.7% for indirect nursing activities and 7.3% for non-nursing activities in medical nursing unit.
In surgical nursing unit she spent the time 58. 7% for direct nursing activities, 36. 2% for indirect nursing activities and 5. 1% for non-nursing activities.
(4) The nurse´s aids spent the time 17. 1% for direct nursing activities, 68. 9% for indirect nursing activities and 14% non-nursing activities in medical nursing unit during the day duty.
In surgical nursing unit, she spent the time 14.5% for direct nursing activities, 78.6% for indirect nursing activities and 6. 9% for non-nursing activities.
These were messengerial work, among the indirect nursing activities mostly.
(5) The ward clerk spent the time 79. 9%, 90.8% for indirect nursing activities in medical and surgical nursing activities respectively.
These were the paper works and messengerial tasks.
3. Patient´s Care Needed.
(1) The patients were provided 58.5%, 51. 1% for direct care by the nursing personnel in medical and surgical nursing unit respectively.
(2) The patients were recieved 13.1°x, 13.5% for direct care by the head nurses in medical and surgical nursing unit respectively.
(3) The patients was recieved 68. 3%, 73.4% for direct nursing care by the staff nurses in medical and surgical nursing unit respectively.
(4) The patients were recieved the direct care of changing the sheet by nurse´s aids mostly. (5) The relatives performed 37.9%, 43.9% for the related nursing activities in medical and surgical nursing unit respectively.
These were preparation, serving a meal and helping the exercise to the patients.
(6) Unmet needs of patient were not significant but showed to be needed helping the exercise.
(7) The average duration of care hours reguired per a day was 33. 2 minutes for class I, 68.8 minutes for class II, and 110 minutes for class III in medical nursing unit. These were in the ratio of 1:2:3.
In surgical nursing unit, the average duration of care hours required per a day was 66.8 minutes for class 1, 114.8 minutes for class II, and 161.6 minutes for class III. These were in the ratio of 2:3:5.
